I want to create a template in confluence that can be used to create a new page. The template would be like a form with text fields. When this template is used to create a new page and there is a submit button on the page, when the page is submitted a JIRA ticket is created with the description field of the JIRA ticket having details of the text field names & values in the confluence page.
Query is:
1. How can I add a submit button to the new page that is created from the template?
2. How to create JIRA ticket on submission of the new page created from the template?
3. How to get the description of the JIRA ticket populated with the text field names & values in the confluence page?

Confluence out of the box doesn't provide the ability to add a button to create a Jira issue to a page template. However, it may be worthwhile to write a blueprint for it. Here are some blueprint tutorials to help your team get started:
