Create a form in confluence, save the data, and retrive the information when searched for.

I'm trying to collect data and create a repository and also search and retrieval of this information. The data has to be retrieved in the format of a form too.

Confluence is a wiki, not a form-processing engine.  If you want to use forms in Confluence, then you'll need an app to layer that functionality over the content.  Take a look at Adaptavist's forms for Confluence, and Confiforms for a couple of good approaches to that.
