Can't see the whole WYSIWYG box, im missing title

I have upgraded from Confluence 3.5.9 to Confluence 6.7.2. Everything seems to work OK except for when i'm trying to add or edit a page. I can't see the title box and I can't resize the box. If you look at the attached image you can see something is not right with the editor box. This also prevents me from creating new pages, when I can't edit the title.

I have tested with different browsers (FF, Chrome, IE and Edge) with the same result.

I understand the editor is not working as expected since upgrading from Confluence 3.5.9 to Confluence 6.7.2.

Please let me know if you followed Upgrading Confluence Manually to upgrade, and whether you upgraded to Confluence 5.0.3 as an intermediate step, as the linked page mentions:

3.5     Upgrade to 5.0.3 then upgrade to the latest version of Confluence 6.

If not, please restore the home directory and database from backup and try the "stepped" upgrade procedure.

Hi,

Yes I followed the guide and used the versions in between. Everything seamed to work except the editing box.
