The end goal is to have a Confluence page with several questions and a button (i.e. "Submit") at the end which will send an email to a predefined address. It will likely be a one time or rare usage scenario and paying for a plugin is not feasible. I don't mind developing something but without plugins such as HTML, EMAIL, FORMS I am unclear on how it could work. Thanks in advance for any suggestions or clarifications.
Not within Confluence no. You could possibly embed some external service using an iframe inside a Confluence page to do it.
However, if you are possibly open to doing it with an add-on I would recommend ConfiForms. I don't work for them, but honestly, it is hands-down the best add-on we have purchased for Confluence and the price is super reasonable.
Is the email address for a distribution list? Is the alert that Confluence sends to those who are watching the page not going to do what you need?

Use case is really a one off form that sends an email upon submit but I don't want to buy a plugin. Just thought there might be something free out there or a crafty workaround which you wouldn't want to do if you did forms all the time but would work for a once ever form.
