Is there a tool (Confluence Macro?) that will show a report or alert for all pages that haven't been updated in over a year? I'd like to use this to trigger an internal review for the page owner to try to eliminate stale and dead pages.

You can set up your own definition of the "Expired" status. In fact, the default "Expired" is set for a page if it has not been updated for 365 days! (in the shot below, the threshold is 100 days)
Then the app will automatically email the last modifier, author, content owner and ask him to update it, confirm that it is still relevant or archive it.
Make sure you give it a try (FREE!) as it can automate the complete workflow and it is extremely customizable.
Worth starting with what is already in the space: Space settings, then Content manager, then the "Last active" filter above the table. It lists pages with no views, no comments and no updates in the period you pick, 180 days by default, and the period is adjustable. For a single space that is usually the answer.
Two limits to know. It works one space at a time, so a site-wide picture means repeating it per space. And bulk archiving or deleting what you find is a Premium and Enterprise feature; on lower plans you act on pages one by one.
If you need the list to live on a page rather than in an admin screen, the filter by label macro puts a live list on a page and takes a space and a last modified date, but it starts from labels, and the pages nobody maintains are usually the pages nobody labelled. CQL through the API covers the rest, at the price of somebody re-running it.

Maybe that will help you out. Otherwise I'm wondering if there might be something with Confluence Cloud Automation, but that's only available for Premire/Enterprise sites, and we're not one of them so I can't check.
