One of my favorites is the 5 Whys template.

A mentor of mine first told me about the 5 Whys methodology many years ago and I've used it ever since. As an admin and a consultant I often have people come to be and tell me the solution that they want rather than the problem they need to solve. 

This template really helps guide focus on getting to the root of the problem and exploring why it needs to be solved.
