Successful Merge. Pull request remains Open.

When using bitbucket's web interface to merge a pull request, we frequently see that the Pull request stays open.  When I look at the Activity for a pull request, I see Merged followed immediately by Opened.  

 

This happens frequently but not all the time.   Has anyone else encounter this?   Do you have any suggestions on how to prevent it?

 

Our default merge strategy is Squash if that makes a difference.
