Is there any way to automatically remove all approvals from all reviewers from a pull request if a subsequent push is made to the source branch?

You might need to create a custom hook for this (https://developer.atlassian.com/stash/docs/latest/how-tos/repository-hooks.html)

Thanks for the link but I should have mentioned that we're just using bitbucket not stash.

Then the answer is most likely no as there are no add on points for Bitbucket.
