Can't add image to Readme.md. Tried other solution still no go

I'm trying to add a local image to my Readme.md file with zero success. Can't believe I'm wasting time on this simple thing.

This is what I'm using

I'm getting that image icon instead. 

I've read so many posts about this, tried so many "work arounds" and nothing works. Why is this so difficult? Can someone help me out here please...
