How do I get notifications when a colleague fills out a form?

I set up a form on Confluence through the Forms for Confluence add-on, but I can't figure out how to set up notifications when one of my team members has filled out a form. I tried adding Slack notifications for edits on the page that displays the forms but it doesn't show up.

Just to share my experience, I made use of ConfiForms IFTTT macro for a similar scenario.

However, this is a add-on macro, so you need to check if you have this existing under your Confluence macro list.
