I've been searching and reading and getting my head turned around with chicken-and-egg procedures with workflows and approvers.
What's the basic steps to set this up? Setup approvers first, then workflows, then Service Request with Approvals?

You should look at the workflow first. Set up at which status(es) you need approvals and by whom.
Once you have that, figure how you're going to populate your approver(s). Is it a single-user, vs. multiple-user, vs. a group.
Then once you have that, associate your workflow to the issue type. You can always use the Service Request with Approvals system-defined issue type or make your own.
