Custom Lookup Field

I'm using an add-in that allows me to map JIRA fields to custom fields in Zendesk.  There are two native JIRA fields that I cannot access when setting up the mappings.  I would like to map the "Team" field and "Ticket Create Date" from JIRA to custom fields in Zendesk.  Neither one of these fields show up as fields that can be mapped with this add-on.  So I'm looking to create a custom JIRA lookup field that looks up the value in both the Team and Created Date and returns the value to the custom field.  Is there a way to do this?
