Refined has an app for customizing the service management portal for self-hosted JSM.
Is there a reason you are looking for a similar app rather than just using that app?
Hello friend, Yes there is a reason, some bug that I could not solve after an update (version 4.22.4 to 4.22.6)
when the user clicks to open an issue the request form looks like this, as in the image. I couldn't solve it to go back to normal where the fields to open an issue appear on this screen:

I haven't found any other apps that provide the same functionality.
Did you confirm this occurs in all browsers, after clearing cache, and for multiple people who should have access to edit the form?
