Post-mortem meeting | Questions to ask new Jira users

I'm reaching out for some advice on how to best source feedback leading to our post-mortem meeting from our new Jira users in our project about the ease & usage of using Jira in a recent technology testing project.  

Ideally, the project lead would like to send a short survey asking their new Jira users on what worked and didn't work when using Jira, and source feedback on areas of improvement.  The questions are purely about their use of Jira and not about the content of the project itself.

Does anyone have a list of helpful survey questions project leads can ask their new Jira users after a project completion about their usage of Jira and areas of improvement?
