Unable to edit form or create fields

We are in a team managed project and are using forms for requests.  I am an admin, but I did not create any of the forms originally. 

We need to change/edit a few fields and add new fields to the forms, but I am unable to do so.  I can create a new form, but I am limited to the fields already created.  

How can I edit existing fields and create new ones?

Hello @tracey.fudge 

Can you confirm you are working with a Work Management project? It is Work Management if you see navigation options across the top for items like Summary, Board, List, Calendar, and Timeline.

What happens when you try to edit a form?

What types of fields are you trying to add to a new form? In what way are you prevented from adding them?

Yes I am working in a Work Management Project.  See attached:

My only option at the form is to delete the form not edit it. Also the only fields I can add are the ones on the right that are already created. I cannot edit the fields. They are grayed out.

There is no Edit option to select to edit a form. You simply start editing it by adding the available fields to it (drag and drop), removing fields from it (by dragging them from the form over to the field list on the right. You should be able to edit the Title of fields on the form by hovering near the title and then clicking on the pencil icon.

The fields available for the form depend on the type of issue the form is connected to. That is shown in the upper left corner of the form. You can add only fields that are relevant to that type of issue. If you need additional fields for the issue type, so that you can add them to the form, you would create and add those fields through Project Settings > Issues Types.
