Dear community,
I would like to create a new Request type: SO new joiner access request. I would like to add the following questions:
Mr/Mrs/Ms: drop down menu
First name: short text
Last name: short text
Email address: short text
Function: short text
Name of Maneger: short text
Sourcing Office: drop down;
Product category : drop down menu
User to be deactivated (if applicable): short text
Customer: short text
Tool: drop down menu
I do not manage adding theses new fields without impacting the CREATE ISSUE SCREEN.
I dont want to see these fields on the CREATE ISSUE SCREEN, but only in this request type in the customer portal.

A request type is associated with an issue type. I would create an issue type for the request that you are putting together in the service desk portal. You can then create a screen scheme / issue type screen scheme and associate the proper screen with only the issue type you would like those fields to show up on.
