A few weeks ago I noticed forms in my tickets started to be formatted differently. This persists on different browsers and after clearing cache. Functionally it works, and when editing an attached form it looks normal again. But when its closed it goes back to looking wrong. I'll attach screenshot
I too have noticed a change in display when viewing a form both in the portal and within the issue view.
Previously when we had a field in the form which was a checkbox or radio button, the form would display all of the available options as well as the selected option. Now the form only seems to show the selected option(s) as text. Where multiple are selected the form now shows each option concatenated together separated by a comma rather than all of the available options with the appropriate checkboxes ticked.
This has made some of our approval processes that we had tied to these forms harder because the approvers cannot easily determine what was and wasn't selected and included on each form.

Do you have a screenshot of this form in original (when formatting is not broken), and if it indeed is different this is the UI issue that needs to be fixed by the developers.
