I have read through a number of suggestions on how to add the 'attachment' option to default field selection but the screen examples do not look anything like the screens I am seeing. I am using a test version of JIRA Service Desk as we are evaluating it for use/purchase for our company. I have been set up as an administrator. Perhaps this is not something available to test versions?
This is what I see as far as my screen config for the test service desk project I set up:

Any assistance would be greatly appreciated.

Hello Beth,
Depending on which version of JSD you are using, the configuration may be a little different. Here is how I would add 'Attachment' as a option for Field Configuration on a JSD project.
Option1:
Option2:
I hope you find this helpful...
Thanks Thomas. I tried both option 1 and 2. Here are my results:
Option 1 - I see the following list of screens for my test project:
I select the default screen scheme and click on edit and I am presented with the following panel:
I was expecting to get a list of the fields.
Option 2:
I am presented with a panel that shows that Attachments are available as a field but when I try to add it to my issue type, it only shows me 8 fields and attachment is not one of them:

Looks like in the 3rd image, attachment is already a screen. It may just not be displayed on the user side. Go to your Project Settings > Request Types and then select the Request Type that you want to associate the Attachment Option with. Next, click on Edit Fields. See below.
You should be able to add the Attachment Option.

The last think I can think of (if that doesn't work) is to go to Project Settings >Issue Types and click on the Field Configuration for whichever issue type you'd like. Once you do that, go down to the Attachment Field Configuration and make sure the action is set to Show. Once this is complete, repeat what I stated above.

Hi Thomas,
I was hoping once we applied permanent license file which we just purchased, it would allow me to see what you show in your screen shots. But, does not appear to be the case. We are running JIRA Service Desk as well as Core. When I look at the versioning, it appears I am running:
You will notice in my 4th screen shot from post last Wednesday, it does NOT reflect 'attachment' as an option in my list of fields to select from. But my earlier screen shot shows that as a default system field.
I am open to other suggestions.
Thanks,
Beth
