Custom forms with a logo

Hello 

Does anyone know if you can create a custom form in Jira that embeds a few items and makes it a pdf?

We need a logo on the top of the form.

official watermark on the bottom 

and capture the fields on the form to create a PDF to send into the comments on our Portal. 

is there a way with a plug in or ?
