For the moment the Slack form is not customizable, but we will explore improvements to the Slack integration in general.

Hi @Hermance NDounga , just wanted to raise this again as our product team raised this as an issue - They are seeing a lot of irrelevant fields and i cannot re- arrange them in a way that only what's mandatory is upfront. 
This improvement could really help internally.
