When I preview the 'Create' form for my board it looks like this:
However, when a contributor clicks 'Create' it appears like this:
Is there a setting somewhere that could have gone awry, or is this a bug?

What @Erin Mihalik said is correct:
Currently this experience only shows if you open the create dialog when viewing the discovery project. If you open it from other projects then it reverts to the standard Jira creation experience.
This is not perfect by any means, but is one of the intermediary steps we took to ship this. Eventually it will all just be one creation experience that works the same across Jira, discovery projects and all.
@dcaddy where are people creating the create button from?
Currently this form only opens if people are clicking on it while in a Jira Product Discovery project (known limitation that we'll address later - not a simple one)

@Tanguy Crusson it is possible for different roles to have different forms for creating ideas though isn't it? ie if a specific template is chosen when Allowing contributors to create ideas - they'll see something different to creators?

The second one looks like Create was accessed while not on the Jira Product Discovery project. That is the Create screen when you are on any other project or atlassian page.
