What is the Needs Fixing state in Crucible and how do i get a review into it?

I know what the other options in the custom filter section of the Reviews Dashboard are and how to get reviews into all states.

But I cant work out what the "Needs Fixing" state filter is based on, or how to get a review into the right state that it would show as needs fixing.

"Needs Fixing" is a review whose state is unknown and not recognized by Crucible. It should not happen. If you find a review in this state it indicates a programming or data issue. The review moderator can put the review into a known state if this happens.

Thanks, would be good to have this in the documentation as "Needs fixing" is open to so many interpretations, the obvious one being there are defects that need fixing!
