There are a few answers available for crucible's version 2 and 3; however they do not work for crucible 4. Moreover, the documentation page returns an erro 503. Is the product being discontinued?
It seems like since version 3.3.2 email subjects are not customisable in FishEye/Crucible, I can see open improvement request: https://jira.atlassian.com/browse/CRUC-6783 though it didn't attract too many votes so doesn't seem higly demanded.

I voted for future users, however I gave up the attempt to use crucible - didn't fit my needs. I'll use UpSource, which works out of the box (in crucible I had to implement a bot to create reviews based on commits, which I think is a basic feature).
Moreover, I found how to change the subject reading all templates. The subject is in the "shared" folder.
Regarding the broken link in the documentation, I didn't find it again - sorry.
