I've been following the Data Centre road-map, paying particular interest in the adoption of forms in Jira Service Management. As you may/may not know this is now a feature in Cloud and I had hoped it would come in an update to Data Centre in due time along with some other tweaks to Service Desk.
However the issue in the road-map has for some reason been removed and I can't see it any more. Does anyone have any updated on whether this has been cancelled?
I'm was trying to avoid using a plugin if this capability was coming however if it's been binned indefinitely I might have to.

Yes Proforma is the plugin I'm referring to.
I'm certain I had seen it in the Data Centre roadmap until recently, see below:

I was hoping this would be included in in DC as I'd rather not have to procure another plugin for what is already adopted in Cloud and should be native in DC.
However if it's been decided it won't go ahead I have no choice but to purchase the plugin.
For now, at least. Yes, a separate purchase is required.
If you're able to adopt cloud that's the ideal solution. However, it's understood some teams must remain on Data Center.
Fingers crossed the app will be included as a part of DC in the future.
