Recently I went to a number of confluence pages and noticed changes to the template that were rolled out w/out our request. We had an outdated header and formatting on a select few pages that were recently deployed more broadly (and yet not universally). I'm trying to understand how to roll it back. Any guidance would be appreciated.

Templates only act at the time of page creation. Changing a template has no effect on existing pages. So I suspect you'll find the pages affected by the changes were all created using the new templates after the change was made.
To roll back, you'll just need to edit the templates to make them do what you had before.
