is it at all possible to create within a form the option of when insert page is pressed a pop up box pops up requestion information which is then also inserted within the page?

Not a direct solution, but a form based data input is possible if you use templates along with the Scaffolding plugin. With this when a page is created using the template it opens up like a form and you can enter the data as per the fields defined.

The confluence version compatiblity with this plugin has to be checked though.
