Where can I find the Contact Administrators form?

This question is in reference to Atlassian Documentation: Configuring the Administrator Contact Page

"Screenshot: The default 'Contact Site Administrators' message"  ...  all very well, but what is the URL to the Administrator Contact Page?  I need to be able to give this out to users who want to request access to Confluence (probably via the site banner).  I am staggered that such a comprehensive guide does not tell me where to find this page.
