Regulated industries require to have a document control process in place that also extends to forms and templates. This includes the need to keep a revision history.
While this is a core functionality for regular confluence pages, this seems to be missing for page templates.
Do I have overlooked something or is this really missing in confluence? Is there an addon or some other means to achieve this?

Unfortunately this is not an existing feature in Confluence. There is an open ticket with Atlassian - CONFSERVER-5876, requesting for the same. You may want to add your vote to the ticket.
