I would like to place a "Create from Template" Macro on Page A, and have it create a new page as a child of Page B. Is this possible? I see that you can define what space the new page is created under, but I cannot find a way to specify page.

Our Page Tree Creator adds this kind of feature to Confluence. So you can create a page tree with buttons on the created pages and configure parameters like: which template to use, which label to add, which target page to use as parent page, etc.
