Is it possible create intake form in confluence that can collect data and store same on confluence

I am currently working on automate my team intake process using confluence, I have created the intake form to collect data and send to different recipient  through outlook but am looking for ways to save/store/document same data in confluence for tracking purposes so that the sender and receiver will be able to track work progress in one page.

There are a couple of different ways to do this.  

1. App from the marketplace - Something like Coniforms
2. You can use the Confluence API to submit the values on a page - https://developer.atlassian.com/cloud/confluence/rest/v2/intro/#about 
3. You can use Jira Service Management to collect the information and an automation to create the Confluence page.
