How do I create a group/distribution list?

Can you give us more details about what kind of list do you want to create? I mean, are you thinking in a email distribution list or something related to user list from Confluence. 

If you want to get a user list from confluence, maybe the User List Macro can be useful for you:

In case you are thinking about a distribution list into your email server, you can create a dummy user with read-only access and give it the email address of the list.

Basically you should add an user into Confluence and then setup as its email, the email address of the distribution list.
