This issue started after the upgrade in Confluence.
I run a weekly meeting and store my notes/actions in those meeting notes. When I start a new meeting, I copy the meeting notes of the previous meeting, delete all the action items, and re-title the notes.
What is happening, is that in the Action Item list, I have duplicate action items. When I click on the meeting notes, there is no action item listed (but it shows on the Action List there is is an action item) It is listed on the original meeting notes but not the one I copied. When click on the action item to complete, it gives me an error message, or the action item button won't accept that the action item is completed.
Again, this all started after the upgrade, I have been doing the meeting notes list this for over 1 1/2 years.

This behavior is a known bug that is being worked on by Atlassian. You can check the report and engage with it here:
Currently, there is no workaround to this behavior. The best approach would be to vote and watch the report. Let us hear from you!
I have the exact same issue. This is affecting productivity of my team as we're using this feature quite a lot!
