Creating an "Is this page helful?" submit form in Confluence

I'd like to create a form that asks readers if the page they're reading is helpful. Ideally, we'd include a Yes and No Option, as well as a Submit button. We'd communicate with a PHP file which stores responses submitted by the form. The form would live on each page, ideally being inserted via a macro (eventually). 

Yes: Submits a Form_Yes notation to the PHP file 

No: Submits a Form_No notation to the PHP file 

I'd also like to include an option for readers to supply more details depending on their selection.

So we'd also include these to be inserted to the PHP file: yes_no_text, or yes_with_text

no_no_text, or no_with_text

Each "...with_text" option would include the feedback as well as the response. The reader would click the Submit button to send the response.

After Submit was clicked, we'd send the info to the PHP file, and then display a "Thank you" note on the page.  We were thinking it could be an HTML form POST method.

Is this possible now? Are there any plans for doing something similar in the future?
