Can I add radio buttons to a page template form in Confluence?

We're using Confluence to regularly collect data from our team, by asking them to fill in forms in Confluence from a page template.  The form fields allow for input through text, multi-line text, and drop-downs.  Is it possible to use radio-buttons somehow, or other options e.g. sliding scale?

Really appreciate any help!  We are currently on Confluence Cloud.

By default, Confluence doesn't have radio button for multiple choices.  Try using this add-on. Open a ticket support for the cloud team to see if they can install it for you.

Thanks very much for your answer.  Since the add-on is only available on Server, how do I open the ticket support to request it for Cloud?  Appreciate your help.
