The source view and latest history showing the two different code base though the commit is same

We are frequently facing GIT issues where source view and latest history are showing diffrent code base and incorrect code base being picked during deployements.

eg. a developer has incorrectly committed the wrong code and after re-checkin as well, the code at times dont rectify at source branch.

History and source view appear different , any idea on this issue?
