How to create an issue using a Jira forms

Jira forms offer a practical way to collect structured information without endless email threads, missing attachments, or untracked requests. Online forms are easy to access, simple to complete, and far more reliable than ad‑hoc messages or spreadsheets.

However, form collection alone doesn’t move work forward.

Tools like Google Forms can gather responses, but once the form is submitted, teams still need to manually review, triage, and recreate that data inside Jira. This gap between data collection and action is where most request processes slow down.

While tools like Google Forms can aggregate data, they do not move the process forward. This is where the combination of online forms and Jira can be powerful, allowing for a seamless transition from data collection to creating issues in Jira.

Creating a Jira ticket based on a submitted form is a crucial process for organizations that use Jira for their work and management. This process ensures that issues form and requests are tracked and addressed in a timely and organized manner.

By integrating  forms in your Jira workflows, organizations can establish a smooth flow from form creating to tracking issues. The Smart Jira Forms is a game-changer in terms of streamlining the form submission process. With this add-on you can easily view and manage form submissions within the corresponding ticket.

Creating a Jira issue directly from a submitted form ensures that every request is:

  • captured in a structured way

  • routed to the right project and issue type

  • immediately visible on Jira boards

  • ready for prioritization and automation

With Smart Forms for Jira, forms don’t live outside Jira — they become part of your Jira workflow.

A submitted form can:

  • create a new Jira work item automatically

  • populate Jira fields (summary, description, priority, labels, custom fields)

  • attach files directly to the issue

  • remain visible inside the issue for collaboration and audits

This works across Jira Software, Jira Service Management, Jira Work Management, and Jira Product Discovery.

External users can create Jira issues — securely

One of the biggest limitations of native Jira forms is access. Traditionally, only Jira users or JSM portal users can submit requests.

Smart Forms for Jira removes that barrier.

Forms can be shared:

  • via direct public link

  • embedded on a website or Confluence page

  • shared from an existing Jira issue

Creating Jira issues from submitted forms helps teams move faster, stay organized, and avoid manual handoffs. Requests arrive structured, visible, and ready for action — whether they come from internal teams or external users.

By centralizing form intake directly inside Jira, Smart Forms for Jira reduces miscommunication, improves transparency, and enables scalable request workflows across teams and departments.

If your current forms stop at data collection, it might be time to let them actually create work.

Not only Jira Service Management (JSM) projects and Jira users can access the form but anyone with the link can submit the form. So, even external users could fill out the form and a new issue in Jira  based on the form fields in the selected project will be created.

The form can be completed and submitted in just a few clicks, and the resulting ticket will be immediately visible in your Jira board.

Creating a Jira issue based on a submitted form is important for a number of reasons:

Ensures that the issue is properly documented:

By creating a Jira issue, all relevant details about the issue are documented in a structured way, making it easier for the team to understand and address the data.

Improves communication:

By creating a Jira issue, a history of the issue is maintained, including any updates, comments, and actions taken. This can be useful for future reference or for identifying trends in support requests. Smart Jira Forms offers a visible form within Jira ticket, displaying all customer responses for efficient collaboration.

In addition to creating custom forms, the add-on also features advanced capabilities like tracking form submissions and exporting the results in .xlsx format, streamlining the process of tracking forms and issue workflows for teams.2023-03-30_13-30-33.gif
