i have built a risk assessment form based on 10 questions and used a scripted filed to calculate the risk value. It worked very well for my users.
Advice avoid allowing a None value in input select lists or check for null in your code.
my calculation was based on select index I.e 0,1,2 etc which meant select list can’t be reordered with amending the script
