When a user fills out a Form on the Customer Portal, the form data is attached to the resulting Jira ticket. I need to access the specific field values entered inside that portal form (programmatically or via automation).

The Forms REST API
You have multiple options.
If a Form field is linked to a Jira field, you can read the value from the normal Jira issue REST API / Jira smart value.
If it is an unlinked Form field, the submitted value belongs to the Form itself and you should use the Forms API.
