Does JSM come with the premium plan? or is that something you have to purchase separately?

JSM is not include when you have a Premium subscription for Jira.
While the two products can co-exist on the same site, JSM (Jira Service Management) is a separate product from Jira and must be purchased/licensed separately.

Yes, you have to purchase the Premium plan for all Atlassian products separately. They are different subscriptions from the Free, Standard, or Enterprise subscriptions/plans.
