Download information of the forms in CSV format

I want to find a way to download the data that requestors have entered in the forms by type of request, in order to be able to extract information from all tickets.

I want to download the information in CSV format, just to highlight that it is not the information of the request, but the information of the forms.

Please can I ask, what is your reason for wanting to export the information from Jira?

It should be possible, for example, for you to have a dashboard with different gadgets to show which tickets were opened by type of request.
