How can you add an attachment option in a table built in a form?

We would like to be able to add attachment in a table built in a form. Here is an example of what we would like this to look like. 

Here Step # would be a multiline text field, along with columns 2/3/4/6/7; column 5 
Test Case Status would be a drop down field. The last column would be a multiline text with the ability to add screenshots. 

Is this option available?

On JSM when editing a FORM, you can add the attachment field.
