Always found the error page when use the plugin Agile Story Mapping in JIRA 6.4.1 (server)

I installed a Trial version of the Agile Story Mapping 2.9.2 to my test server for an evaluation. However, when I play around the plugin, it very often occur the error page as the attachment which I cannot continue the evaluation. I tested it to 2 servers and found the same issue. Could you please advise the root cause of the problem and how to solve it?

Information: I use Chrome browser and JIRA Server 6.4.1.

please try latest Agile User Story Map v2.10.1 - download from Marketplace:
