how to connect crucible and JIRA???

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!
I have installed crucible. When I open crucible I get the connection window.
I provided everything correct but still I get the following error. I would be really happy if someone provide me a solution or explain step by step process from installing to code review using crucible an JIRA.
