Can't Open Code Review Crucible

Hi,

I just install Crucible the trail version.  However I can't open it.  When I open it from windows it opens 2 dos windows and then closes.  I open the web version but I don't have create review tab in the web version.  Would you be able to guide me how to create a code review with Crucible?
