To expand just a bit, 4.2.1 is currently available. All you need to do is schedule the maintenance window via a support request.
Enterprise Hosted customers are given the choice to request for upgrades anytime they want it and it is available. There is no automatic update. However, if you opt for OnDemand, it would be done periodically anytime there is a new release.
