Weighted Questionnaire

I want to create a questionnaire that will be empty each time a user goes onto it and i want the questions to be weighted so I can give a final score at the end.

The plugins I've seen so far don't allow for the questions to be weighted or allow for the questions to be fresh for each user or load up. Are you able to recommend anything that would help with this?

An edit on this, a better word I could use would that I want to create a test/exam for them to answer and give feedback on their answer immediately.

@Qais Khan There is nothing like what you are asking for in the Atlassian Marketplace.  I would suggest looking for a third-party application that you could either embed or link to our of Confluence.
