I believe an upgrade to the Confluence cloud app built with Forge UI automatically updates all the installation. However, does an individual user has to approve/accept again in order to get the upgrade.
NOTE: The app uses Confluence User object to store some metadata (secrets).
Is this expected or any alternative approach to bypass user re-approval (grant permissions) again.

You have go through the "Approve access" interaction only if the new app version requires additional permissions! If it doesn't, then there is no AA.
For example, you installed and approved app version 1. But then the developer added a new feature which requires an additional permission, which was not required by app version 1. If he releases this as app version 2, then after updating to version 2, the user has to approve it again.
It does make sense.
Also, note that Atlassian plans to remove the whole AA in the next months. Theoretically it is a correct thing to do, but it is really cumbersome.
