When generating native release notes from Jira to Confluence, is it possible to use a custom template rather than the default template?

Hi @Julia Watson-Clarke it doesn't appear this functionality is available with the base product at this time. I found JRACLOUD-87058 out on Atlassian's Jira Site requesting this functionality.
Feel free to vote for and watch it for updates.

If you are open to using a marketplace app (a free one at that) you may want to take a look at Automated release notes & reports for Jira. It will let you not only create Confluence pages based on Jira events (such as version releases, sprint completions and more) but also update, prepend or append to them.
Free version is more than sufficient for small teams who have the need to run 5 such automated actions per month.
