On form submission create a new confluence page with data entered in the form

Is it possible to create a Form with submit button, and when user enters details in this form and submits a new page will get created, this page will have data user entered in the form.

Use case - Collection of user requirements and creating a requirement document on each request.

Its basically passing on variable values from one page(Form) to another (New page)
