Odd behaviour using Create from Template to call a user created template

We are using Confluence 6.11.1 in our organisation

I have tried to copy the content of the Decision template to a new template and have created a page that includes a "create from template" macro that uses my new template to create a new page. 

When using the Decision blue print selecting Create Decision shows this view;

But when I use Create from Template on another page linked to my new template I get:

As far as I can tell my new template and the Decision template are the same. Can someone tell me why my template does not work the same as in the confluence Decision log blueprint

Also can anyone explain what the "placeholder" variables are for on the Decision template
